Well the main cause of this issue is because of the feature that allows installing apps on to the SD directly, a feature found in Froyo (Android 2.2) and upwards. If a widget is installed on a SD card, it will be not loaded again on the next reboot.

So a rather simple workaround is to place the concerned app in the Phone Memory, using the following steps:

1. Go to Settings > Applications 
2. Select Manage applications
3. Select the concerned app (the widget app.) Here we’ve  selected Astrid Tasks app.
4. Click on the Move to Phone button
5. That’s it

If it again happens, just reinstall the app once and you will be good to go on forever.

This does happen because the apps are installed onto the SD card directly or either by using an app like App2SD.
